If you only get one field right, make it this one. Most of the reports that teach anything are not about a substance — they are about a combination, and the combination hides in the things people do not think to mention.
The usual omissions, and why each matters
- Fluoxetine (Prozac): half-life of days to weeks. Someone who "stopped it last week" is still a CYP2D6 inhibitor and still serotonergic. The most under-reported interaction on any experience board.
- Grapefruit / Seville orange: a CYP3A4 knock-out that lasts ~a day — spacing it out does not help.
- Alcohol and nicotine: everyone forgets to list them. Alcohol + kava is a documented liver hit; nicotine changes a lot of pharmacokinetics.
- Caffeine: a competitive substrate and, in quantity, a cardiovascular variable in its own right.
- "Natural" supplements: St John's wort (a CYP3A4/P-gp inducer), piperine (an inhibitor), high-dose anything.
How to write it so it is useful
List the dose and the LAST time taken, not just "I take X". Half-life is the hidden variable: a drug taken 36 hours ago can still be fully active. If you take a regular medicine, name it even if you are sure it is unrelated — "sure it is unrelated" is exactly how interactions get missed.
A report that says "nothing else" is making a strong claim; make sure it is true before you type it.
