This board borrows the one thing the experience vaults got right — structure — and asks you to fill in the field that decides whether a report is data or an anecdote. Copy this skeleton. Say "unmeasured" where it was unmeasured; that is data, not a confession.
What was taken
Substance or preparation, form, amount, and route. Amounts in real units where you have them (mg, ml, g). If it was a plant, say which part and how it was prepared.
What else was in the system
Everything, including the boring things: prescriptions, caffeine, alcohol, nicotine, grapefruit, supplements, and anything in the previous 72 hours with a long half-life (fluoxetine is the classic). This field is the whole point of the board — most of the reports worth reading are really interaction reports.
Timeline
Clock times from the first dose. Onset, peak, notable turns, resolution, next-day.
What happened
Effects, and separately your interpretation of them. Keep the two apart.
What would have been worth checking first
With hindsight — which isoform, which interaction, which test. Link the page: a mechanism question on Pharmacology & Interactions, a reagent result from Herb.
What I would change
One line.
A report that fills "what else was in the system" honestly is worth ten that skip it. Safe-storage and labelling discipline for anything dosed: https://wiki.soapbox.community/wiki/Testing_Labelling_and_Safe_Storage_of_Dosed_Preparations
